Clinical review Observational studies as alternatives randomized clinical trials in surgical clinical research

چکیده
CLINICAL EPIDEMIOLOGY AS A DISCIPLINE has evolved rapidly dur ing the last decade. However, application of epidemiotogic principles in surgical clinical research has been limited. In the March 1996 issue of SURGERY we discussed the principles, application, and limitations of randomized clinical trials (RCTs) in surgery. RCTs are often viewed as the gold standard for evaluation of therapeutic efficacy because they are the closest that clinical medicine can come to the controlled environmen t of laboratory research. However, clinical decisions often have to be made in the absence of data obtained this way. In this issue we will discuss some alternatives that can be used in surgical clinical research. Alternatives to RCTs fall into the purview of observational epidemiology, which can be grouped into five major categories: case reports, case series, cross-sectional studies, case-control studies, and cohort studies. The former three are often referred to as descriptive studies, and the remaining two are labeled as analytic or comparative studies.

Refractory iron deficiency anemia and Helicobacter Pylori Infection in pediatrics: A review
Background Since the discovery of Helicobacter pylori, several clinical reports have demonstrated that H. Pylori infection has emerged as a new cause of refractory iron stores in children. We carried out a systematic literature review to primarily evaluate the existing evidence on the association between childhood H. Pylori infection and iron deficiency anemia (IDA) and secondly, to investigat...

Background & Aim: Randomized controlled trials often suffer from two major problems, i.e., noncompliance and missing outcomes. One potential solution to this problem is using the intention-to-treat (ITT) analysis approach. Therefore, the aim of this study was to review the concept of ITT and the most important issues related to it in practice since RCT researchers utilize it as a guide in order...

Therefore, this study was conducted toevaluate the effect of fennel on primary dysmenorrhea. Methods: This systematic review was conducted on clinical trials (non-randomized, randomized, historical study with co...
متن کاملEvaluation of Clinical Trials
In a number of important clinical issues such as evaluation of the efficacy or effectiveness of therapeutic or preventive interventions as well as for comparing the harms of interventions, randomized controlled trials (RCTs) provide the highest levels of evidence, either directly or indirectly. It is obvious that critical appraisal of these studies to assess their validity and precision is of p...

Evidence-based medicine (EBM) is a shift in medical paradigms and about solving clinical problems, acknowledging that intuition, unsystematic clinical experience, and pathophysiologic rationale are insufficient grounds for clinical decision-making. The importance of randomized trials has been created by the concept of the hierarchy of evidence in guiding therapy. Even though the concept of hier...
